DATA PROTECTION STATEMENT
We are asking that you consent to Gerard Clarke Consultant Gastroenterologist Ltd. keeping data that relates to you for the purposes of providing you with medical care. The data will be shared with hospitals including; hospital admission departments, Insurance companies, general practitioners and other medical and/or para-medical professionals involved in your care.
The data that we hold identifies you and includes; contact information, insurance details, marital status and next of kin and relevant medical history. We will also maintain records of medical services we provide including clinic notes and reports of investigations including endoscopy, radiology and pathology reports. Communications from general practices (GP) relevant to the care we provide are also maintained.
We will only keep your data in so far as is necessary to provide you with medical care or to deal on your behalf with insurance companies etc.
We will hold your data until seven years have elapsed from your last treatment/consultation with us or 7 years after you have reached the age of 18 years which ever is later – or you instruct us to delete your data. In rare circumstances your data maybe processed outside of the European economic area and if this is the case we will take steps to ensure that the same standards of protection apply as would within the European economic area.
